Student Checklist (You will need the following to enroll):

• Photo Release Form (to be completed at the Academy)
• Proof of Residency (cable, utility or phone bill) *
• Proof of Custody (if applicable) *
• Income Reduced Breakfast-Lunch Form (if desired)
• School Registration Form (to be completed at the Academy)
• Birth Certificate *
• Social Security Card (or copy) *
• Immunization (Shots) Records (or copy) *
